Here is a simplified 10-step suggested claim process for preparing a claim. Simple 10-step process PDF You will also need an initial claim registration form: FORM 1 Initial registration form with instructions (PDF format for printing out) FORM 1 Initial registration form with instructions (Word format for on-screen completion) If you need further assistance in preparing your claim please e-mail to info@kresy-claims.org Request for extension of time In order to assist you we have drafted a standard letter to the Claims Office seeking postponement of a decision on your claim for 3 years to give you more time to get the information they have requested. See here. The letter is in both English and Polish so English speakers who are not fluent in Polish can read the letter. However, you must send the Polish version. If you wish to request a postponement you should add in: 1. Your address details; 2. The address of the relevant Claims Office; 3. Your claim number; 4. Any changes to the body of the letter; and 5. Sign it and send it. You must send this letter before the 6 months from the date of the Claims Office letter requesting information has expired. If anyone receives a negative response from the Claims Office please let us know. It may also be possible (though we don't know for sure) to request a further postponement before the 3 years is up. However, any requests will have to sent sent before the 3 years has expired. In special acts of support, the Consulates General of the Republic of Poland in Toronto (Canada), Sydney (Australia), London (UK) and New York (USA) have arranged to accept claim applications and to send them to the proper office in Poland. However, according to Polish law, ALL consulates are considered delivery points for an application as long as it is submitted to the consulate prior to the end of the deadline. For consulates that do not seem aware of this, you may quote the Polish law Kodeks Postepowania Administracyjnego http://www.lex.com.pl/serwis/du/2000/1071.htm , Article 57 paragraph 5: "Termin uwaza sie za zachowany, jezeli przed jego uplywem nadano pismo w polskiej placówce pocztowej albo zlozono w polskim urzedzie konsularnym." (a deadline is met when the application is posted at a Polish Post Office or lodged in a Polish Consular post.) The date of submitting an application to the Consulates will be recognized as the date of admitting it by the office in Poland. Therefore it is possible to meet the Dec. 31, 2008 deadline by getting an application to the Consulates before that date.
